Dr. Rayner graduated from the University of Utah School of Medicine and completed his residency training in adult psychiatry at the University of Nevada, Reno. He is a member of the American Psychiatric Association and the clinical TMS society. He specializes in the prevention, diagnosis, and treatment of mental disorders, emotional disorders, psychotic disorders, mood disorders, anxiety disorders, substance-related disorders, sexual and gender identity disorders, and adjustment disorders.
Dr. Rayner clinically focuses on working with individuals and families experiencing serious mental illness. As a medical officer in the Utah Army National Guard, Dr. Rayner has several years of experience and a true passion working with soldiers and veterans in our community.
Dr. Monica Polk is a dedicated psychiatrist at Whole Mind, where she provides comprehensive mental health care for children, adolescents, and adults. Her services include psychiatric evaluations, medication management, and psychotherapy. Dr. Polk is deeply committed to helping individuals and families navigate complex emotional and behavioral challenges with empathy, evidence-based care, and respect.
Dr. Polk earned her Doctor of Osteopathic Medicine degree from the University of New England College of Osteopathic Medicine. She completed her psychiatry internship at Hartford Hospital’s Institute of Living in Connecticut, followed by an adult psychiatry residency at Thomas Jefferson University Hospital in Pennsylvania, where she was recognized as Resident of the Year for inpatient psychiatry. She went on to complete a Child and Adolescent Psychiatry Fellowship at Children’s National Medical Center in Washington, D.C., and a Forensic Psychiatry Fellowship at the University of Maryland Medical Center, where she became board-eligible in forensic psychiatry.
Her diverse background includes extensive work in clinical, academic, and forensic settings. Dr. Polk’s expertise spans child and adolescent psychiatry, trauma, mood and behavioral disorders, and forensic evaluations. She is a member of the American Academy of Psychiatry and the Law and remains passionate about bridging the gap between mental health and the legal system to ensure compassionate, informed care.
Dr. Polk is driven by a deep belief in the importance of mental health and strives to provide every patient with thoughtful, comprehensive, and individualized treatment.
Judy is a Marriage and Family Therapist with a passion for helping individuals, teens, and families heal from trauma, rebuild trust, and reconnect—to themselves and to each other. She brings a grounded, compassionate presence to her work, always meeting clients with authenticity, insight, and deep respect. As someone who actively practices the skills she teaches, Judy models the kind of growth and integration she helps others pursue.
Her approach is systems-based and trauma-responsive, drawing on evidence-based modalities including EMDR, DBT, and Attachment-Based Family Therapy. Clients appreciate her thoughtful, down-to-earth style and her ability to hold space for both vulnerability and transformation.
Judy earned her Master’s in Marriage and Family Therapy from Northcentral University, where her capstone focused on Virginia Satir and transformational systems therapy. She has extensive experience in residential and school-based mental health, including work with adolescents and adults in dual diagnosis treatment, crisis response, and community care. Whether she’s leading equine-assisted therapy or walking with clients through family recovery or substance use healing, she brings steadiness, clarity, and a sincere belief in each person’s capacity for change.
She is also a lifelong advocate for social justice, with an academic background in peace and justice studies. Judy received an award for her conference submission and presentation on immigration at the 2010 J. Bonner Ritchie Dialogue at Utah Valley University, where she was part of the honors program. With past roles as both a director of operations at an equestrian center and a therapist in private and public settings, she offers a rare blend of wisdom, grit, and warmth.
Outside of work, you’ll find Judy spending time with family, exploring nature, enjoying all things horses, or dreaming about her next beach trip. She believes therapy is both an art and a science—and that with the right support, everyone can create meaningful change.
Riddhi is a graduate of Lincoln Memorial University where she earned her Master of Medical Science degree in Physician Assistant Studies. She attended Mercer University for her undergraduate studies where she studied Biology, Chemistry, and Religion. She is a member of the American Academy of Physician Associates and is committed to being a lifelong learner. Her passion for psychiatry stems from a strong desire to help positively impact the lives of those around her.
Riddhi is extremely passionate about medicine and its ongoing advances and hopes to holistically provide her patients with the best care. She hopes to serve as an advocate for her patients so that she can help them collaboratively advance their mental health and well-being. Outside of work, Riddhi values quality time with her family and friends. She also enjoys traveling, cooking, hiking, yoga, and giving back to the community.
Pam Major is a Licensed Clinical Social Worker with extensive experience supporting adults across the lifespan in navigating trauma, anxiety, depression, grief, PTSD, identity development, and faith transitions. She specializes in working with neurodivergent clients (ADHD and Autism), members of the queer community, and couples seeking deeper connection and healing.
Pam’s therapeutic approach is compassionate, collaborative, and grounded in evidence-based care. She draws from a broad range of modalities, including Narrative Therapy, Acceptance and Commitment Therapy (ACT), Emotionally Focused Therapy (EFT), Eye Movement Desensitization and Reprocessing (EMDR), Internal Family Systems (IFS), Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Mindfulness-Based Therapy. She places particular emphasis on the role of personal history and narrative in shaping emotional well-being, integrating this understanding throughout the therapeutic process.
Pam is dedicated to providing inclusive, affirming care, creating a safe space for clients whose experiences are often marginalized or misunderstood. Her work is informed by a trauma-responsive, neurodiversity-affirming, and LGBTQ+-affirming framework.
Outside the therapy room, Pam enjoys time with her two kids, exploring mountain trails, and following her dog’s lead on off-trail adventures. These moments keep her grounded in the values of connection, presence, and play.
Chaleena graduated with a Bachelor’s degree in Social Work and minors in Psychology and Art Education from Weber State University. She completed a Master of Social Work degree at Boise State University and is licensed as a Certified Social Worker.
Chaleena has experience working with patients from many demographics with various mental illnesses and has training in therapeutic modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Acceptance and Commitment Therapy (ACT), Motivational Interviewing (MI), Solution-Focused Therapy (SFBT), and Mindfulness Interventions.
Chaleena is passionate about empowering clients and provides a strengths-based approach to therapy. In her free time, Chaleena enjoys being outside on nature walks or a paddleboard, creating artwork (especially painting), and spending time with her dog.
Carissa is a board-certified Psychiatric Mental Health Nurse Practitioner with a Master of Science in Nursing from Frontier Nursing University and over 13 years of diverse nursing experience. Her career began in psychiatric nursing, expanded to high-acuity ICU care, and most recently included serving as a charge nurse in labor and delivery.
Carissa’s advanced training encompasses comprehensive mental health care for diverse populations, with a special focus on perinatal mental health. She is passionate about providing patient-centered care and is a dedicated advocate for her patients, empowering them to achieve balance and well-being in their lives.
Outside of work, Carissa enjoys cooking, traveling, and cherishing time with her husband and three children, creating memories that fuel her passion for life and care.
Jenn is a highly accomplished Nurse Practitioner with over two decades of experience as a registered nurse. She has dedicated her career to providing exceptional care to her patients. Six years ago, she fulfilled her lifelong dream by earning a Doctorate in Nursing from the University of Utah, showcasing her commitment to advancing in the medical field.

With three years of specialized experience in women’s health, she has become a trusted expert in this crucial field. At Whole Mind, she utilizes medication-assisted treatment when appropriate and focuses on destigmatizing substance use through education, therapy, and compassionate care.
Anne’s path to psychiatry was deeply personal. She believes no one should feel shame around their mental health and works to equip each person with the tools they need to thrive.
Outside of her work, Megan loves spending time with family, hiking, and traveling with her husband. At home, she enjoys quiet evenings with a good Netflix show and her cat by her side.
Jordan Zabriskie, LCSW, is a licensed clinical social worker with more than a decade of experience helping individuals, couples, and families navigate life’s challenges and strengthen emotional well-being. He holds dual master’s degrees in Social Work from Pennsylvania Western University and Behavioral Science from the University of Cincinnati, bringing both academic depth and practical expertise to his work.
Jordan specializes in treating depression, anxiety, ADHD, trauma, PTSD, borderline personality disorder, and recovery from narcissistic or emotional abuse. He works with children (ages 10+), adolescents, adults, and members of the LGBTQ+ community. His therapeutic toolkit includes CBT, DBT, ACT, MBT, Psychodynamic, Somatic, and Mindfulness-Based therapies—allowing him to tailor each session to the unique needs of his clients.
Fluent in English, Spanish, and Portuguese, Jordan is known for his emotionally attuned and compassionate approach.
